What is cornea?
The cornea is the clear, dome-shaped surface that covers the front of your eye. It plays a critical role in focusing your vision. Various conditions can affect the cornea, requiring specialized care and treatment.

Symptoms of corneal conditions
Recognizing the symptoms of corneal conditions early can lead to prompt treatment and better outcomes. Common symptoms include:
- Blurred or Distorted vision.
- Eye pain or discomfort.
- Redness or Swelling.
- Increased sensitivity to light (Photophobia).
- Excessive tearing or discharge.
Causes of corneal conditions
Corneal conditions can arise due to a variety of factors, including:
- Some corneal conditions, like keratoconus, may run in families.
- Bacterial, viral, fungal, and parasitic infections can affect the cornea, often due to improper hygiene or eye injuries.
- Trauma from foreign objects, chemical exposure, or accidents can lead to corneal damage.
- Conditions like rheumatoid arthritis can cause inflammation affecting the cornea.
- Exposure to UV rays, wind, and pollutants can contribute to corneal issues.
- Poor lens hygiene or overuse can increase the risk of infections and other corneal problems.
Complications of corneal conditions
- Severe conditions may lead to vision impairment.
- Open sores can cause pain and vision issues.
- Damage or infection can affect vision clarity.
- Risk of the immune system attacking a new cornea.
- Persistent pain or irritation.
- Compromised cornea may lead to infections.
Common corneal conditions treated
- Dry eye syndrome: A condition where the eyes do not produce enough tears or the right quality of tears, causing discomfort and blurred vision.
Management includes artificial tears, punctal plugs, and lifestyle adjustments. - Pterygium: A benign growth of tissue on the cornea, often related to UV exposure. It can cause discomfort and vision issues. Surgical removal is the common treatment for significant cases.
- Corneal infections (Keratitis): Infections caused by bacteria, viruses, fungi, or parasites. Symptoms include redness, pain, and blurred vision. Treatment varies from antibiotic drops to more intensive therapies based on the infection type.
- Keratoconus: A progressive condition where the cornea thins and bulges into a cone shape, leading to distorted vision. It can be managed with specialized contact lenses, corneal cross-linking, or corneal transplantation in advanced cases.
- Corneal dystrophies: A group of genetic disorders where abnormal material accumulates in the cornea, affecting vision. Types include Fuchs’ dystrophy and granular dystrophy. Treatments may involve medications, corneal cross-linking, or transplantation.
- Corneal scarring: Caused by injury, infection, or inflammation, leading to vision impairment. Treatment options range from medications and therapies to corneal transplantation, depending on the severity.
Advanced diagnostic techniques of corneal conditions
Modern diagnostic tools have significantly improved the ability to diagnose and treat corneal conditions accurately. Key techniques include:
- Corneal topography (Pentacam): A non-invasive imaging technique that maps the surface curvature of the cornea, useful for detecting irregularities like keratoconus. It is one of the pre-operative test before any refractive procedure.
- Optical Coherence Tomography (OCT): A high-resolution imaging technique that provides detailed images of the cornea’s layers, aiding in the diagnosis of diseases like Fuchs’ dystrophy.
- Tests for dryness
- Tear Breakup Time (TBUT) test: Measures the stability of the tear film to evaluate dry eye severity.
- Schirmer test: Assesses tear production by measuring the amount of moisture on a small strip of paper placed inside the lower eyelid.
- Fluorescein staining: Uses a dye to identify damage to the corneal surface and assess the extent of dryness or other issues.
- Tear Breakup Time (TBUT) test: Measures the stability of the tear film to evaluate dry eye severity.
Cornea Treatments
Dry eye management
Dry eye is a common condition that occurs when your eyes do not produce enough tears or the right quality of tears. It can cause discomfort, burning, and a gritty sensation. Treatment options for dry eye include:
- Artificial tears: Over-the-counter or prescription eye drops that help lubricate the eyes.
- Punctal plugs: Tiny devices inserted into the tear ducts to block drainage, keeping the eyes moist.
- Medications: Prescription eye drops that reduce inflammation and help the eyes produce more natural tears.
Care:
- Environmental adjustments: Avoid air conditioning or heaters that dry out the air; consider using a humidifier.
- Diet: Increase omega-3 fatty acids intake, which can help improve tear quality.
Recovery:
- Timeline: Symptom relief may be gradual, depending on the severity of dry eye.
- Ongoing management: Dry eye is usually a chronic condition requiring ongoing management.
Pterygium surgery
A pterygium, or “surfer’s eye,” is a non-cancerous growth on the cornea that can cause irritation, redness, and vision problems if it grows over the pupil. Pterygium surgery is performed to remove this growth and prevent recurrence, typically using minimally invasive techniques that reduce healing time and discomfort.
Care:
- Eye drops: Use antibiotic and anti-inflammatory drops as directed to prevent infection and reduce inflammation.
- Minimize exposure: Avoid dusty or smoky environments that can irritate the healing eye.
- Rest: Take it easy and rest your eyes for a few days after surgery.
Recovery:
- Timeline: Initial recovery is usually within a few weeks, with complete healing taking a few months.
- Scar tissue: Some redness or scarring at the site is normal and will gradually fade.
Treatment of corneal infections
Corneal infections, or keratitis, can result from bacteria, viruses, fungi, or parasites. Symptoms may include pain, redness, blurred vision, and sensitivity to light. Early diagnosis and treatment are essential to prevent serious complications. Treatments can range from antibiotic or antifungal eye drops to more intensive therapies, depending on the cause and severity of the infection.
Care:
- Medication compliance: Strictly adhere to the prescribed regimen of antibiotic, antiviral, or antifungal eye drops.
- Hygiene: Maintain excellent hand hygiene to prevent reinfection or spreading the infection.
- Avoid contact lenses: Refrain from wearing contact lenses until your eye care professional advises it's safe.
Recovery:
- Timeline: Recovery varies depending on the severity of the infection but typically spans a few weeks.
- Follow-up: Regular check-ups ensure that the infection is resolving and prevent potential complications.
Corneal collagen cross-linking
Corneal collagen cross-linking is an advanced treatment primarily for keratoconus. This minimally invasive procedure involves applying a special riboflavin solution to the cornea and then exposing it to ultraviolet (UV) light. The process strengthens the collagen fibers within the cornea, helping to stabilize its shape and halt the progression of keratoconus.
Care:
- Avoid eye rubbing: It’s essential to avoid rubbing your eyes as the cornea heals and strengthens.
- Sunglasses: Wear sunglasses outdoors to protect your eyes from UV light.
- Pain management: Use recommended pain relief if you experience discomfort or sensitivity.
Recovery:
- Timeline: Most patients notice improvement in their vision within a few weeks to months.
- Healing process: Mild discomfort and blurry vision are common during the initial healing phase.
Corneal transplantation
Corneal transplantation, or keratoplasty, is a surgical procedure used to replace a damaged or diseased cornea with healthy donor tissue. This is often recommended for patients experiencing conditions such as:
- Keratoconus: A condition where the cornea thins and bulges into a cone-like shape, causing distorted vision.
- Corneal scarring: Often caused by injury or infection, leading to vision impairment.
- Corneal Dystrophies: A group of genetic eye disorders where abnormal material builds up in the cornea.
Types of corneal transplants include:
- Penetrating Keratoplasty (PKP): Replaces the full thickness of the cornea.
- Deep Anterior Lamellar Keratoplasty (DALK): Replaces the outer and middle layers of the cornea while preserving the innermost layer.
- Descemet’s Stripping Endothelial Keratoplasty (DSEK): Involves transplanting only the innermost layer, ideal for specific conditions like Fuchs' dystrophy.
Each type of transplant has its own indications and benefits, and an eye care specialist can help determine which is best for your needs.
Care:
- Medication: Use prescribed eye drops to prevent infection and reduce inflammation.
- Eye protection: Wear an eye shield or glasses to protect the eye from injury and dust.
- Avoid strain: Limit activities that could strain your eyes, like heavy lifting, bending over, or vigorous exercise.
Recovery:
- Timeline: Recovery can take several months. Vision improvement may be gradual, depending on the type of transplant.
- Follow-up: Regular check-ups are crucial to monitor healing and detect any signs of rejection early.

Prevention tips for corneal conditions
- To reduce eye strain, take a 20-second break to look at something 20 feet away every 20 minutes.
- Blink regularly to maintain a healthy tear film and reduce dryness.
- Wear UV-protective sunglasses.
- Clean lenses regularly and handle them properly.
- Avoid eye injuries.
- Treat allergies to reduce eye irritation.
- Get regular check-ups.
- Use artificial tears.
- Follow lens guidelines.
- Stay hydrated and eat well.
- Avoid rubbing eyes.
Contact lens service
Contact lenses are a popular alternative to glasses for vision correction. They offer a broader field of view and greater convenience for many people. Whether you need lenses for daily wear or specialized lenses for specific conditions, it’s important to find the right type for your eyes.
A proper contact lens fitting is essential for comfort and eye health. During a fitting, an eye care professional will measure the curvature of your cornea, assess your tear film, and check your eye health to determine the most suitable contact lenses for you.
Ortho-K involves wearing specially designed contact lenses overnight to reshape the cornea temporarily. This non-surgical option corrects myopia (nearsightedness) and allows for clear vision during the day without the need for glasses or contact lenses.
Some individuals have specific vision needs that require specialty contact lenses. These include:
- Rigid Gas Permeable (RGP) lenses: These lenses are more durable and offer clear vision for people with astigmatism or irregular corneas.
- Scleral lenses: Larger lenses that rest on the white part of the eye (sclera) and vault over the cornea, ideal for individuals with severe dry eyes or corneal irregularities.
- Toric lenses: Designed specifically to correct astigmatism, providing a stable and clear vision.
Proper education on how to wear and care for contact lenses is crucial to avoid complications like infections. Eye care professionals provide instructions on inserting, removing, and cleaning your lenses to ensure your eyes remain healthy.

Hello friends I am glad to inform you about Dr Dhwani that I was very worried about doing cataract removal surgery on both eyes as my elder brother who is mentally ill for the last several years and he is undergoing cataract surgery on both eyes. Dr. Dhwani Maheswari explains this cataract operation in a very simple and effective manner to the young child in a very simple and effective manner in a situation where he was not ready for this and was not ready for his personal medicine and treatment. As soon as he fell unconscious, he was operated on cataract and I was afraid that he himself is mentally ill and if he does not pay attention, if he falls asleep, who will take care of him? In this situation, the way he performed this operation very effectively after one week and the fear that I had inside me that if this brother does not pay attention, then his eyes will go away, today by giving the necessary treatment to my brother who is mentally ill. Today they can read without any problem and there is no problem.
I cannot thank Dr Dhwani Maheswari enough for this. Thank you very much.
